5. List sources to which applications for funding for this project have been made, and
amounts of requests and commitments. Indicate with an asterisk those sources from

6. Describe future funding plans if this is to be an ongoing project.
7. Please list any grants previously received from the Minnesota State Bar Foundation
giving period during which funding was received and amount (s).
8. Please attach the following:
(a) Overall organization budget as well as a specific project or legal program budget indicating items for which Bar Foundation support is requested. You may attach a brief narrative explanation of the budget if necessary.
(b) A copy of the most recent Form 990, or Form 990T if applicable, with all schedules submitted to the IRS and/or an explanation of the organization’s fund balance including restricted and nonrestricted funds.
(c) If applicable, and only if you have never applied for a Bar Foundation grant before or if the original status has changed, please attach the IRS statement of nonprofit status.
(d) If the applicant proposes a cooperative venture or partnership with any
other organization (s), letters of support from the organization (s) must
accompany the proposal.
This application will not be considered unless the entire application is completed with attachments and the Foundation’s evaluation form for a previous grant has been completed. The evaluation may accompany your submission.
