The Elder Care Guide contains the following resources that are helpful quality of care indicators for consumers when looking for nursing homes and other care providers. The resource listing includes links to: results of periodic facility surveys; findings of complaint investigations that have been conducted; and characteristics of nursing homes.

    • “Nursing Home Compare” website operated by the U.S. Dept. of Health & Human Services for people receiving Medicare at:    
      • View details of past performances of nursing homes
    • “Nursing Home Selection” website co-sponsored by the Minnesota Dept. of Human Services (MDHS) and the Minnesota Dept. of Health (MDH) at:
      • View nursing home report card that ranks homes in 7 quality measures and sorts based on input from consumers on priorities for care
    • Contact the MDH Information & Compliance Division and request a summary report of deficiencies found during surveys from the Centers for Medicare and Medicaid Services (CMS) Online Survey Certification and Reporting System (OSCAR), called “Oscar Reports” at:

    • “Assisted Living Reviews” periodically conducted by MDH at:
      • View results of MDH surveys of facilities conducting business as “assisted living” facilities, which includes facilities providing services under a Class A or Class F license in a housing with services establishment and meeting statutory minimums
